An Individualized Education Program (IEP) is a written statement for each child with a disability that is developed, reviewed and revised at least annually that identifies and provides:

A statement of the Present Level of Acadmic Achievement and Functional Performance
-
A statement of Measurable Annual Goals
-
A description of how and when progress toward meeting the annual goals will be measured and reported
-
A statment of services that are to be provided to enable the child to be involved and make progress in the general education curriculum
-
An explanation of the extent the child will participate with nondisabled peers
-
A statement of accommodations necessary to measure academic achievement and functional perfomance on State and district-wide assessments
-
The projected date for the beginning of services including frequency, location and duration
-
Transition services to be considered and in effect not later than when the child turns 14, or as determined by the IEP team
